To schedule an appointment or for more information about any of our services call (770) 551-4350 to reach our Atlanta office, (706) 453-3383 to reach our office in Greensboro or (866) 343-8061 for our Emory John’s Creek office. To cancel an appointment, please notify our office at least 24 hours prior to your scheduled appointment to avoid a $25.00 no show charge.

If you are a new patient and have already scheduled your appointment, you may download our registration forms ahead of time and bring them with you to your appointment. To view and download forms you will need a copy of Adobe Acrobat Reader installed in your computer. Directions

From I-85 take I-285 West toward Chattanooga/Birmingham. Take the Ashford Dunwoody Road exit. Turn right off ramp onto Ashford Dunwoody Road. Turn left at the first light onto Hammond Drive. Go through two lights. Turn right at first drive (after the second light) at the HealthSouth sign. Take first left into parking lot and look for the green awning.

From 400 Northbound (from Atlanta) exit Glenridge Drive and take a right at the light. Go until it deadends into Peachtree-Dunwoody Road and turn left. Go about two miles to Hammond Drive. Turn right onto Hammond Drive. Take the second left into parking lot and look for the green awning.

Insurance and Billing

Prior to your procedure date, you will be notified of your estimated insurance coverage and amount of co-pay due. (Due to the number of insurance plans, we suggest that you contact your insurance company if you have a specific question regarding your coverage.)

After surgery, we will file your insurance papers for you. Any amounts not covered by your insurance and deductibles will be collected on the day of surgery. We accept Visa, MasterCard, Discover and American Express with credit card approval on the date of service.

It is your right as a patient to request information or have questions answered regarding fees or payment policies. If you have any questions or problems regarding you bill, please contact our business office. We will be happy to help you between the hours of 8:30 am and 4:30 pm, Monday through Friday.
